Several insurance companies participate in YMCA membership discount programs. Please contact your insurance provider to see if the Madison Area YMCA is a partner.
As of November 1, 2011, UnitedHealthcare insured members who join or are current members of the Madison Area YMCA, and record a minimum of 12 visits per calendar month will be eligible for reimbursement through the OptumHealth Program and Healthy Contributions.
The eligible UnitedHealthcare Group member, with the Activation Letter stating their unique Activation ID in hand, may enroll as a Madison Area YMCA member according to our rates and policies. Current YMCA members can enroll for reimbursement by printing out and bringing their own OptumHealth Activation Letter to the Y. The YMCA is responsible for tracking the member’s visits per month. Members must continue to pay membership dues to the Madison Area YMCA and seek reimbursement from United Healthcare.
OptumHealth has contracted with Healthy Contributions to reimburse $20 to each member ($40 for a couple/family) who meet the threshold of 12 visits per month. Healthy Contributions will reimburse members directly into checking or savings account, on or after the 25th of every month. The Madison Area YMCA is not responsible for reimbursing the member. Madison Area YMCA members MUST check into the facility by scanning their card, in order to track their visits. Signing into the visitor log book will not count as a visit. Only active memberships in good standing will record/report visits.
All members and potential members interested in participating in this reimbursement program must print out your activation letter to determine eligibility, fill out the 2 OptumHealth forms as well as the Madison Area YMCA Membership forms. If they are current members they just need to fill out the 2 OptumHealth forms available at the Welcome Center.
